REPLACEMENT OF THE RESIDENCE CARD

 

A foreigner is obliged to replace their residence card in case when:

  • 1) the data presented in the document change;
  • 2) the appearance of the person changes and hinders the establishment of their identity;
  • 3) the card is damaged to the degree that prevents its use;
  • 4) the card is lost or destroyed;
  • 5) the validity of the residence card issued in relation to obtained residence permit for EC long-term resident has expired.

 

BODY REPLACING THE RESIDENCE CARD

 

The residence card is replaced by a voivode with territorial jurisdiction for the foreigner's place of residence. Residence card is issued and replaced after the due fee has been paid.

 

An application for card replacement is submitted using a form. A foreigner applying for residence card replacement is obliged to submit a valid travel documents and attach the following to the application:

  1) photographs of persons covered by the application;

  2) documents necessary to confirm the data and circumstances entered in the application.

 

In particularly justified cases, when a foreigner applying for the replacement of a residence card does not have and cannot obtain a valid travel document, they may present another document confirming their identity.
