LEGAL BASIS
Articles 42, 43, 45 and 63 of the Act of 14th July 14 on the entry into, residence in and exit from the Republic of Poland of nationals of the European Union Member States and their family members (Journal of Laws no 144, item 1043, as amended)..
COMPETENT AUTHORITY
In order to register one's stay in the Republic of Poland or obtain a residence card, an application must be submitted at the Voivodeship Office competent for a place of residence of an EU citizen.
IMPORTANT INFORMATION
A STAY NO LONGER THAN 3 MONTHS
An EU citizen may enter and stay in the territory of the Republic of Poland no longer than 3 months on the basis of a valid travel document or other valid document confirming his or her identity and citizenship.

FEES
A fee for issuing a certificate of registration of an EU citizen's stay shall amount to PLN 1.
When collecting a certificate of registration of an EU citizen's stay, an EU citizen is obliged to present a valid travel document or another valid document confirming his or her identity and citizenship, and a proof of having paid the fee of PLN 1; in case where the certificate is collected by the applicant's representative - a power of attorney to collect the certificate must be presented.
APPEALING
A foreign national may appeal the decision to the Head of the Office for Foreigners through the intermediary of the competent Voivod. Written appeals may be submitted within 14 days from the day of receiving the decision.
REPLACING A CERTIFICATE OF REGISTRATION OF AN EU CITIZEN'S STAY
Certificate of registration must be replaced in case:
In case of a loss of a certificate of registration of an EU citizen's stay, a new certificate is issued.
Replacement of a certificate of registration of the EU citizen's stay or issuing a new certificate will be done without delay after an EU citizen has submitted his or her application.
The application shall be submitted in person. The requirement of personal submission of an application does not concern minors.When submitting an application requesting the replacement of a certificate of registration of the EU citizen's stay or issuing a new certificate, the EU citizen is obliged to present a valid travel document or another valid document confirming his or her identity and citizenship.
