LOSS OF THE RESIDENCE CARD
A foreigner who has lost their residence card is obliged to notify the voivode who has issued it within 3 days of the date when it was lost. In case when a lost residence card is found, a foreigner is obliged to notify the voivode within the 3 days of finding it and immediately return the found residence card if the lost card has already been replaced with a new one.
A foreigner is obliged to immediately return the residence card to the body that has issued it after receiving a document acknowledging that Polish citizenship has been granted or a decision on withdrawal of residence permit for EC long-term resident.
